Proteomic Analyses Lead to a Better Understanding of Celiac Disease: Focus on Epitope Recognition and Autoantibodies

Abstract  Proteomic technologies are being used with increasing frequency in the scientific community. In this review, we have highlighted their use in celiac disease (CD). The available techniques, which include two-dimensional (2D) gel electrophoresis, mass spectrometry, and antibody and tissue arrays, have been used to identify proteins or changes in protein expression specific to gut tissue from patients with CD. A number of studies have employed proteomic methodologies to determine the diagnostic biomarkers in body fluids or to examine changes in protein expression and posttranslational modifications during signaling. As our knowledge has evolved, we now recognize that celiac disease, the most common form of malabsorption, is a specific autoimmune disorder resulting from permanent intolerance to gluten, the main storage protein of wheat and other grains. The disease was once believed to result from toxic or allergic effects induced by a deleterious agent but is now known to be a T-cell–mediated inflammatory disorder characterized by an immune response against gliadin. Bone mineral density and importance of strict gluten-free diet in children and adolescents with celiac disease

Conclusion: Children and adolescents on not strict GFD are at increased risk for low BMD. We therefore recommend that BMD should be evaluated in patients with positive EMA. In addition, patients on strict GFD are at risk for low BMD because of low calcium intake or vitamin D deficiency. Therefore, strict GFD with recommended calcium intake and vitamin D supplementation during winter and spring should be encouraged in all children and adolescents with celiac disease. (Source: Bone)
